9-5-5: TREE REMOVAL PERMIT APPLICATION AND FEES:
Removal, relocation or replacement of trees is prohibited unless a permit therefor has been first obtained. Tree removal permits for the removal, relocation, or replacement of trees covered herein shall be obtained by submitting to the Building and Zoning Official an application on a form prescribed by the Village. The tree removal permit fee shall be fifteen dollars ($15.00) for lots containing homes, home units, and developed and occupied commercial and industrial units. The tree removal permit fee for tree clearing in conjunction with the issuance of a building permit shall be twenty five dollars ($25.00).
   A.   Application Procedure: For tree removals in conjunction with new construction on lots without designated building pad or building areas:
      1.   Tree removal permit application not involving a building permit shall include:
         a.   A sketch, photo or other description showing the location of the tree or trees to be removed.
         b.   Information on the size and types of trees to be removed.
         c.   Reasons for removing the trees.
         d.   Any reports or studies, if any, indicating that the trees should be removed.
         e.   Tree survey; see subsection 9-5-5A2g.
      2.   Tree removal permit application involving a building permit shall submit at their own expense, as an attachment to their application, two (2) copies of a legible reproducible site plan drawn to scale and indicating the following information for the entire site:
         a.   Location, shape, and spatial arrangement of all existing and proposed walls, improvements, and structures.
         b.   Identification of uses on adjacent properties.
         c.   Location, shape, and spatial arrangement of all off-street parking and access roads.
         d.   Existing and proposed utility services.
         e.   Existing and proposed elevations.
         f.   Setbacks, yard requirements, and easements.
         g.   A tree survey overlaid directly upon the site plan and indicating the location, referenced to structures, of all trees with a DBH of eight inches (8") or greater. The survey shall distinguish existing trees which are proposed to be destroyed, relocated, replaced, preserved at their present location, or introduces into the development from an off-site source. All trees shall be identified by species and DBH. Residents wishing to obtain a tree removal permit for their own single-family residential property may prepare their own tree survey. Groups of trees less than three feet (3') apart may be designated as clumps, provided that any tree with a DBH of eight inches (8") or more must be specifically designated. The location of all tree removal activity is to be indicated on the tree survey. The Building and Zoning Official may exempt those portions of the site which he determines will not be affected by the development activity from inclusion in said tree survey. (Ord. 96-32, 10-7-1996)
