3-3-7: APPLICATION FOR LICENSE:
An applicant for a license shall submit an application under oath, on forms obtainable from the Clerk stating, among other things, the following:
   (A)   His name and residence address.
   (B)   That he is a citizen of the United States.
   (C)   His place of birth, and if a naturalized citizen, the time and place of naturalization.
   (D)   That he has never been convicted of a felony, or an offense involving moral turpitude.
   (E)   The location where he proposes to sell at retail, alcoholic liquors.
   (F)   That the proposed location is not within one hundred feet (100') of any church, school, hospital, home for aged or indigent persons or for veterans, their wives or children or any military or naval station.
   (G)   That he will not violate any of the provisions of this Code, or the laws of the State of Illinois or the United States, in the conduct of his place of business.
   (H)   In the case of partnership or corporation, the information required by this section shall be furnished as to each partner and the principal officers of the corporation.
   (I)   That he has never been convicted of being the keeper of a disorderly house and that he is not now the keeper of a house of ill-fame.
   (J)   That he has never been convicted of pandering or other crime or misdemeanor opposed to decency and morality.
   (K)   Said application, in duplicate, shall be made to the Liquor Control Commissioner and the Liquor Control Commissioner shall thereupon immediately furnish to the Chief of Police, a copy of said application, and he is hereby directed to make an immediate investigation of the said applicant and to furnish the chairman of the Local Liquor Commission promptly after his investigation, a report of what his investigation discloses concerning the said applicant, which report shall be made a permanent record of the local Liquor Commission and shall be filed together with the application with the Liquor Control Commission to be kept as a permanent Village record.
