§ 150.03 NATURAL GAS LICENSES AND FEES.
   (A)   General. No person, firm, corporation or agent shall hereafter install, alter, repair or service gas burners or gas burner equipment within Forest Lake without conforming to the provisions of this chapter.
   (B)   Licenses required. No person, firm, or corporation shall engage in the business of altering, repairing, or servicing of gas burners without first securing a master gas installer’s license from the Clerk permitting him or her to engage in the business. The requirements contained herein shall not be construed to limit the right of the gas utility and its authorized employees to render necessary service.
   (C)   Liability insurance. No license or renewal shall be granted, nor shall the same be effective, until the applicant shall file with the City Clerk proof of a public liability insurance policy covering all operations of the applicant under this chapter for the sum of at least $300,000 against liability for bodily injuries or death for each person, for the sum of at least $1,000,000 against liability for bodily injuries or death to more than 1 person from 1 accident and for at least $300,000 against liability for damage or destruction of property. The city shall be named and the insurance provided shall include the city as an additional party insured. The policy shall provide that it may not be canceled by the insurer except after 10 days written notice to the city, and if the insurance is canceled and the licensee shall fail to replace the same with another policy conforming to the provisions of this chapter, the license shall be automatically suspended until the insurance shall have been replaced. The policy or policies of insurance shall also contain a “completed operations clause” conditioned for the faithful performance of all work, entered upon or contracted for, in strict accordance and compliance with the terms, requirements, and provisions of all applicable laws, ordinances, rules and regulations and shall be kept in full force and effect for and during the license period.
   (D)   Examinations. The City Clerk may require an examination of applicants for gas installers’ licenses if he or she deems it necessary.
   (E)   Violations. Any person who shall violate any provision of this chapter shall be guilty of a misdemeanor and, upon conviction thereof, be punished according to law, and his or her license granted under this chapter may in such case be revoked by City Council.
