(A) Form. Every application for a license or permit issued under this chapter shall be on a form provided or required by the city. Every application shall state the name of the applicant, the applicant’s age, representations as to the applicant’s character, with references as the Council may require, the type of license applied for, the business in connection with which the proposed license will operate and its location, a description of the compact and contiguous premises where alcoholic beverages may be dispensed and consumed, which may not include any parking lot or sidewalk, whether the applicant is the owner and operator of the business, how long the applicant has been in that business at that place, and other information as the Council may require from time to time. Applications shall be verified and filed with the City Clerk and no person shall make a false statement in an application.
(B) Financial responsibility. Prior to the issuance of any license or permit under this chapter, the applicant shall demonstrate proof of financial responsibility as defined in M.S. § 340A.409 with regard to liability under M.S. § 340A.801 unless such financial responsibility is not required for the particular license or permit sought pursuant to state statute. Required surety bonds of insurance policies shall be subject to the approval of the City clerk. This proof must be filed with the City and the Commissioner of Public Safety. Applicants claiming an exemption to the financial responsibility requirement based on application of M.S. § 340A.409, subd. 4, shall file an affidavit with the City Clerk claiming such exemption. Compliance with the financial responsibility requirements of state law and this subdivision is a continuing condition of any license or permit, and failure to have insurance coverage or a bond as required by this section shall be grounds for immediate revocation of the license.
(C) Fees. Applications for licenses and permits shall be accompanied by the required fee for the license or permit.
