§ 50.05 LICENSED COLLECTOR.
   (A)   License required. No person, corporation or other entity shall engage in the business of collecting refuse, recyclables or demolition and construction waste within the City of Forest Lake without obtaining a business license as provided by this section.
   (B)   Application. Any person, corporation or other entity desiring to be licensed as a collector shall make application to the city on a prescribed form setting forth:
      (1)   The name and address of the applicant;
      (2)   A description of each piece of equipment proposed to be used in the collection;
      (3)   The proposed charges to be made of those who use the service;
      (4)   A description of the kind of service proposed to be rendered and the frequency of the collection;
      (5)   The place to which the refuse is to be hauled;
      (6)   The manner in which the refuse is to be hauled;
      (7)   A description of each type of container that will be used to receive and contain refuse that may accumulate between collections.
   (C)   Insurance. No license shall be issued until the applicant files with the city a current policy of public liability insurance covering vehicles to be used by the applicant in the licensed business. The limits of the insurance shall be not less than $500,000 against liability for bodily injuries or death for each person; not less than $1,500,000 against liability for bodily injuries or death to more than 1 person from 1 accident and not less than $500,000 against liability for damage or destruction of property.
   (D)   Fee. The annual fee for a license shall be in an amount established by duly adopted ordinance.
