145.10  MEDICAL/HOSPITALIZATION INSURANCE.
   (a)   All full-time employees, including full-time employees on probationary status who have been employed by the Village for thirty days, shall be entitled to group medical- hospitalization insurance coverage as provided by the Village of Forest as follows:
      (1)   Married employees or unmarried employees with natural-born or adopted children shall be eligible to receive family or individual coverage.
      (2)   Unmarried employees without children shall be eligible to receive coverage for the individual employee only.
 
   (b)   Regular part-time employees shall not be entitled to receive any coverage at Village expense but may participate in the Village group plan by paying to the Clerk-Treasurer an amount equal to the cost of such coverage.
 
   (c)   A full-time employee who is eligible to participate in the Village's group plan may elect not to participate in the plan provided the employee demonstrates to the satisfaction of the Clerk-Treasurer that he or she is covered by other medical/hospitalization insurance. An employee who elects not to participate in the Village's group plan may receive additional taxable compensation at such rate as Council may choose to establish from time-to-time. An employee who elects not to participate in the Village's group plan may apply for such coverage during the annual open enrollment period or if other medical/hospitalization coverage becomes unavailable.
 
   (d)   The cost of group medical/hospitalization insurance shall be borne by the same fund or funds from which each employee's salary or hourly compensation is paid as set forth in the pay classification and wage schedule ordinance.
