957.04 APPLICATION AND LICENSING.
   (a)   Initial Licensing/Prohibition. No person or entity shall operate an ambulance/EMS within the corporate limits of the City of Findlay without having first obtained a valid license issued by the Mayor’s Office.
   (b)   Application. Application to operate an ambulance/EMS within the corporate limits of the City of Findlay must be made on a form prescribed for that purpose and filed with the Mayor’s Office at least 30 calendar days in advance of operating as an ambulance/EMS. Applications shall include the following:
      (1)   The names, residence and business addresses of the owner, partners, and stockholders of a corporation petitioning to operate such ambulance/EMS.
      (2)   A descriptive list of all vehicles to be operated in such service within the City, including make, model, year and odometer reading of each unit at time of application.
      (3)   The proposed location[s] from which the provider will operate from.
      (4)   The names, residence addresses, and current medical certification[s] of all persons to be employed with the provider of such ambulance/EMS service within the City of Findlay;
      (5)   The name and business address of the Medical Director.
      (6)   Proof of insurance as required by Section 957.03 of this Chapter.
      (7)   Proof of operating and licensing as an “Advanced Life Support Emergency Medical Service.
      (8)   Proof of other fitness and certification, as described elsewhere in this Chapter, including but not limited to proofs of inspection of equipment and review of certifications and other records by the City of Findlay Law Director or his/her designee.
      (9)   Vehicle inspections shall be conducted pursuant to Section 957.07 of this Chapter.
   (c)   Annual Review.
      (1)   The term of any license issued pursuant to this Chapter shall not exceed one calendar year. Licenses shall be in force and effect until January 1st following the date when the license was issued.
      (2)   No later than December 1st in the year when the license expires, the provider must submit an application for license to operate during the succeeding year. The application and documentation requirements are the same as for an initial license application, described at Section 957.04(b) of this Chapter.
      (3)   It shall be the responsibility of the ambulance/EMS to contact the Mayor’s Office to arrange for inspection of vehicles and equipment and filing of required documents.
   (d)   New Vehicles and Equipment. New vehicles and/or equipment obtained during a year in which an ambulance/EMS provider is licensed to operate in the City of Findlay, must be inspected and approved, pursuant to Section 957.07 of this Chapter, before any such vehicle may operate within the City.
   (e)   Newly Assigned Personnel. New personnel who are assigned to work within the corporate limits of the City of Findlay during a year in which an ambulance/EMS provider is licensed to operate in the City of Findlay must provide such certifications as required by Section 957.06 of this Chapter before beginning work in any medical capacity for the provider.
