Information provided shall include, but not be limited to, the following: name and address of applicant, responsible organization, exact location of activity, dates and hours of duration, temporary structures or equipment, sanitary facilities, parking areas, crowd control provisions, food and drink available and any other information needed to determine whether the proposed activity does not jeopardize the health, welfare and safety of the public.
