Application for a certificate of occupancy required by Section 1359.01 shall be made not less than fifteen days prior to such occupancy by the owner or prospective occupant, and shall include the following information:
(a) The street address of the dwelling structure, or specific addresses of the parts or part if the entire structure is not involved.
(b) The name and address of the owner, agent or person in charge, and the phone number where such person may be reached during normal business hours.
(c) In the case of direct sale, or sale by land contract, the name, address, phone number of the buyer and the number of prospective occupants, or in the case of rented premises, the names of all of the prospective occupants, if known.
(d) A statement as to whether the application is being made pursuant to a direct sale, sale by land contract, rental, lease or a specific combination of the same, and the proposed date of occupancy.
(e) A statement by the applicant as to any changes made or proposed to be made in the structure itself, of which he has knowledge.
In the case of premises occupied by other than the owner, the failure to apply for a certificate of occupancy as required by the provisions of this chapter, or the moving into, or commencement of occupancy without such a permit, shall constitute a violation of this chapter by either the owner or occupant, or both.
