1024.02   PERMIT APPLICATION.
   (a)   Each permit for making an excavation or for hauling earth or debris shall be confined to a single project and shall be issued by the City of Fairlawn. The application shall be made on a form prescribed by the Director of Public Service or designee, and shall give the exact location of the proposed excavation, the kind of paving, the area and depth to be excavated, and such other facts as may be requested. Each application shall be accompanied by an application fee of fifty dollars ($50.00).
   (b)   An applicant for a permit for making an excavation in any street, alley, right-of-way, sidewalk or public ground shall, before such permit is granted, make a cash or approved security bond deposit with the Director of Finance as set forth in this chapter.
   (c)   The Director of Public Service or designee may deny a permit application if: the application is not complete; or the proper deposit has not been made; or if the applicant or contractor involved has previously failed to comply with City ordinances and regulations; or the applicant or contractor involved has previously failed to perform in a competent or workmanlike manner on work done in the City; or for such other reasons that the Director of Public Service or designee deem appropriate.
