1309.02 FORM; DEPOSIT.
(a) Each application for a permit, with the required deposit shall be filed with the Building Superintendent on a form furnished by him, and shall contain a general description of the proposed work and its location. The application shall be signed by the owner or his authorized agent. A deposit in accordance with the following schedule for commercial and industrial permits will be required with all applications. This deposit will apply toward the permit fee, except that it will be forfeited if the permit is not picked up within sixty days after the date of application. The deposit amount for commercial and industrial permits shall be as follows:

(b) Each application for a permit shall indicate the proposed occupancy of all parts of the building and of that portion of the site or lot, if any, not covered by the building and shall contain such other information as may be required by the Building Superintendent.
