(A) Report form. The Chief of Police shall cause report forms to be printed to provide for the following information:
(1) The name and residence of the person from whom purchased, to whom sold, by whom pledged, or with whom otherwise dealt;
(2) The age, sex, complexion, color of mustache or beard, style of dress, and height of such person;
(3) The time when the articles were purchased, sold, pledged, or otherwise dealt in;
(4) The date of the report;
(5) The name and place of residence of the person making the report; and,
(6) The hour of the day when the report was made.

(B) Filing of reports. The Chief of Police shall deliver report forms to the licensees free of charge and shall, upon receipt of such reports, file the same in his office. Such reports shall at all times be open to inspection only by the Police Department of the city and the peace officers of the county.
