Permits shall be issued in a form so that they may be affixed to the vending device. Each permit shall contain the following information:
(a) The name and address of the permittee;
(b) A description of the vending device;
(c) A description of the permitted location(s);
(d) The expiration date of the permit; and
(e) If applicable, the location of the commissary which supplies the vending device; and any other information the Director of Public Service deems desirable.
