(a) All applications for certificates of registration, licenses and certificates of qualification shall be in the form prescribed by the person or agency authorized to issue the same or authorized to recommend the granting of the same.
(b) Applications for certificates of registration, for licenses and for those certificates of qualification for which a renewal fee is prescribed, shall be sworn to by the applicant before a notary public.
(c) All applicants must hold a license in a community having an examining board.
