An application for a permit shall be submitted to the Building Department stating the following facts:
(a) The lot number and building number;
(b) A plot plan showing the location of the building on the lot;
(c) The total height and area of the building to be demolished;
(d) The names and addresses of adjacent property owners;
(e) The names and addresses of the owner of the building to be demolished, and also the names and addresses of the persons engaged to perform the actual demolition;
(f) The proposed length of time to complete demolition work; and
(g) An affidavit to the effect that the site will be left in a clean, healthful and nonhazardous condition upon completion of work.
