§ 110.26 APPLICATION FOR PERMIT.
   (A)   An application for permit to run a temporary business under this subchapter shall be made at the city office. Such application shall be sworn to or filed with the city at least three weeks prior to the time in which the permit applied for shall become effective.
   (B)   The application shall contain the following information:
      (1)   The name, address, and/or headquarters of the person applying for the permit;
      (2)   If the applicant is not an individual, the names and addresses of the applicant’s principal officers and managers;
      (3)   The name and address of the person or persons who will be in direct charge of conducting the temporary business;
      (4)   The time within which the temporary business will be operated and the location of the temporary business in the city; and
      (5)   Any application for a temporary permit must also be signed by the party on whose property the temporary business shall be located.
