All permits granted hereunder shall be deemed to include the following conditions and terms, and an application for a permit shall be deemed to be an acceptance on the part of the permittee of the following terms governing the permitted work procedures, which shall be construed as conditions of the permit:
(A) All work requiring workers or vehicles to be located on the pavement or shoulders of the public right-of-way shall comply with the terms of the Ohio Manual of Uniform Traffic Control Devices published by the Ohio Department of Transportation, and any special provisions made by the village and noted on the permit pertaining to safety procedures to be used to protect passers-by and workers at or near the job site.
(B) All work undertaken under a permit hereunder shall be closed and permanently restored in accordance with the standards for construction and materials published by the Ohio Department of Transportation, shown in the cross-sectional plates attached to Ordinance No. 24-90, which are incorporated herein and attached hereto by reference, all of which indicate the materials and methods for restoring various different roadway conditions.
(C) In addition to the foregoing, the following conditions and terms shall be included in all permits:
(1) The permit applicant shall be responsible for all contractors and subcontractors involved;
(2) A copy of the permit shall be maintained with a person at the job site at all times, and shall be shown to village authorities on request;
(3) All work shall be completed and performed without cost to the village;
(4) No materials, tools, or vehicles used to complete the work shall be stored, even temporarily, on the pavement or village right-of-way without the approval of the village to do so, as appears in the permit granted or as subsequently approved by the Street Commissioner;
(5) All areas disturbed by the operation shall be restored to a condition comparable or better than to that which existed prior to the work undertaken;
(6) All sidewalks, steps, driveways, drains, mail boxes, traffic signs and control devices, head walls, poles, fences, and the like that are disturbed shall be restored to their original condition or better;
(7) All pavement and sidewalks shall be kept free of dirt, mud and debris at all times; and
(8) All equipment used on the pavement shall be rubber-tired except for blacktop rollers.
