Each application for a license shall be made in writing to the Mechanical Examining Board at least 30 days prior to the examination date, stating the class of license applied for, the name and place of business of the applicant and the names of the representatives of the applicant responsible for the work to be done under the license. A nonrefundable examination fee as set by the Mechanical Examining Board shall be paid when application is made.
