8-8-2: CONSTRUCTION REQUIREMENTS:
   A.   Permits Required: It shall be unlawful to construct or lay any pavement on any public street, sidewalk, alley or other public way, or to repair the same, without having first secured a permit therefor. Applications for such permits shall be made to the director of engineering and community development and shall state the location of the intended pavement or repair, the extent thereof and the person who is to do the actual construction work. No such permit shall be issued except on order of the Mayor and board of trustees.
   B.   Bond And Insurance:
      1.   Bond: Each applicant shall file a bond in the amount of ten thousand dollars ($10,000.00) with sureties to be approved by the director of engineering and community development conditioned to indemnify the village from any loss or damage resulting from the work undertaken or the manner of doing the same.
      2.   Insurance: Each applicant shall further provide evidence to the village of insurance; said certificate of insurance to name the village of Elk Grove Village as an additional party insured and be sufficient to cover all contract work to be performed by the general contractor and any subcontractor and shall be in the following amounts:
         a.   Comprehensive general liability insurance for each occurrence in the sum of one hundred thousand dollars ($100,000.00) and aggregate in the sum of three hundred thousand dollars ($300,000.00).
         b.   Property damage in the amount of one hundred thousand dollars ($100,000.00) for each occurrence.
         c.   Comprehensive automobile liability in the amount of one hundred thousand dollars ($100,000.00) for bodily injury each occurrence and three hundred thousand dollars ($300,000.00) for bodily injury aggregate and one hundred thousand dollars ($100,000.00) property damage for each occurrence.
         d.   Excess umbrella liability in the amount of one million dollars ($1,000,000.00) to cover claims exceeding the above principal coverages; providing, that such excess umbrella coverage shall not be required if the amounts of the comprehensive general liability insurance and the comprehensive automobile liability insurance as set forth in subsections B2a and B2c of this section are increased to the sum of five hundred thousand dollars ($500,000.00) for each occurrence and an aggregate sum of one million dollars ($1,000,000.00).
         e.   Workers' compensation.
         f.   Employer's liability of one hundred thousand dollars ($100,000.00) each accident.
   C.   Specifications: All street and sidewalk pavement shall be made in conformity with the specifications laid down by the Mayor and board of trustees.
