Permit fees for small wireless facilities shall be as follows:
A. New Utility Pole: One thousand dollars ($1,000.00) for each small wireless facility addressed in an application that includes a new utility pole.
B. Collocation: Five hundred dollars ($500.00) for a single up-front application that includes up to five (5) small wireless facilities, with an additional one hundred dollars ($100.00) for each small wireless facility beyond five (5).
C. Annual Rate: Two hundred seventy dollars ($270.00) per small wireless facility per year is the annual recurring rate to collocate a small wireless facility on an authority utility pole located in a right-of-way.
