An application for a license shall be made to the Village upon forms provided by the Finance Department and shall contain such information as is necessary to ensure compliance with the provisions of this Article and with the provisions of the revised rules, regulations, and standards prescribed by the Department of Public Health, State of Illinois, as hereinafter adopted by reference. All information contained on said application must be current and any changes therein must be reported to the Finance Department within two (2) weeks of any such change.
