A. Village License: It shall be unlawful for any person to conduct, maintain, operate, carry on or manage an ambulatory surgical treatment center without first having obtained a license as is herein provided.
B. State License: In addition to the license requirement contained herein, each center must be licensed by the Department of Health of the State of Illinois under the Ambulatory Surgical Treatment Center Act - Public Act 78-227, approved July 19, 1973.
