§ 130.63 REQUIRED INFORMATION.
   All applications shall be submitted according to the special event permit application (Appendix A). This document is subject to change at any time. The application will be critical in evaluating the citizen impact and public safety related to the event. Every representative shall submit a completed application, which shall contain the following information:
   (A)   The name of the event;
   (B)   The type of the event (concert, charity event, county fair, and the like);
   (C)   A general event description (what is being offered, who is the target audience, and the like);
   (D)   Contact information for the manager and the sponsoring agency;
   (E)   Contact information for the onsite manager(s) during the event;
   (F)   Documentation confirming the sponsoring organization's status (formal/informal; profit/not-for-profit);
   (G)   The expected attendance;
   (H)   A crowd control plan;
   (I)   The location, site layout, and parking areas;
   (J)   Planned structures (temporary or permanent);
   (K)   Whether vendors will be serving or cooking food;
   (L)   Any planned voice amplification or music;
   (M)   Planned electrical power sources
   (N)   Any planned hazardous materials;
   (O)   Alcohol sales information (subject to ABC Board approval);
   (P)   All dates that the special event will operate at the given site. Dates not initially specified shall be subject to the acquisition of an additional permit;
   (Q)   A traffic control plan, including any proposed street closure or traffic flow;
   (R)   Any other specific services requested;
   (S)   Information on county and fire support services that may be required from:
      (1)   Security/law enforcement;
      (2)   Fire/rescue;
      (3)   Standby emergency medical services;
      (4)   Waste cleanup; and
      (5)   Any other services deemed necessary by the town, such as Public Works and the County Health Department.
   (T)   A first aid plan;
   (U)   A weather contingency plan;
   (V)   Pedestrian access and flow;
   (W)   The number of comfort facilities (restrooms) that will be accessible. Restroom capacity must meet or exceed the requirements of the North Carolina State Building Code;
   (X)   Trash and recycling facilities/provisions;
   (Y)   Written consent of the owner if different from the sponsor; and
   (Z)   Any other information reasonably required by the Town Manager’s office to determine the nature or scope of the event.
