Each applicant for a taxicab license shall present and file with the Manager an application, which shall contain the following:
(a) The name of the applicant and of each officer, partner or business associate;
(b) His or her present occupation and place of business;
(c) His or her place of residence for five years preceding the application;
(d) The location of the intended place of business;
(e) The trade name under which he or she intends to do business;
(f) The number of vehicles for which a license is desired, with a description of each;
(g) The marking or lettering to be used on each taxicab; and
(h) Any other information required by rule or regulation of the Manager.
