(A) No person shall make a connection to the water facilities of the village without first making application to the village for the privilege of making the connection and to have a written permit approved by the Director of Public Works and to pay all applicable fees.
(B) (1) Persons desiring the connection to the water facilities of the village shall, in addition to making application, fully state all purposes for which water is required and answer all questions put to them by the Director of Public Works relating to the proposed water consumption. In case of fraudulent representation on the part of the user, the payment will be forfeited and the use of the supply of water stopped.
(2) No permit will be issued unless the permittee:
(a) Pays the capital connection fees as prescribed in this chapter;
(b) Pays the cost of the meter;
(c) Secures a street opening permit in conformance with the provisions of Chapter 91 of this code of ordinances;
(d) Agrees to pay all costs of the installation including the connection to the water main;
(e) Agrees to give the Director of Public Works 24 hours notice prior to the start of the
installation of the service and when the service shall be ready for inspection, except that no inspections will be made at times other than during regular village working hours;
(f) Agrees to pay all cost of inspection if such inspection is deemed necessary by the Director of Public Works;
(g) Agrees that the materials used and the method of installation shall be in conformance with specifications established by the village;
(h) Agrees to conform to all rules governing plumbers and to the regulations of the Illinois State Plumbing Code; and
(i) Agrees to secure and protect the village from any liability or damage whatsoever for injury, including death, to any person or property and files with the village liability insurance certificates.
(C) All underground water connections plumbing works shall be done subject to the approval of the Director of Public Works and no work underground shall be covered up until examined by him, her or some person under his or her direction.
(D) No water shall be turned on to a connection until meters are installed and the applicant shall have paid the fees due. All plumbers are strictly prohibited from turning the water on to any connections unless first permitted to do so by the Director of Public Works. This shall not prohibit any plumber admitting the water temporarily to test the pipes and plumbing work for which purpose only the water may be turned on.
(E) Water metes are to be billed to customers at the village’s then current cost.
(F) A fee, as established by the annual fee schedule, shall be charged for termination of water service which shall include turning the water service back on.
