A. Approval of participation in a major trunk line extension shall be by contract between the city/authority and the applicant. This contract shall prescribe, at a minimum:
1. The basis and extent of city/authority participation in the construction of the facility;
2. The basis and extent of applicant participation in the construction of the facility, including any additional costs for contract services required to accomplish construction of the facility;
3. The applicant shall secure the design of the major trunk line extension in accordance with local and state requirements;
4. Participation is subject to submission and approval of plans, specifications by the city/authority and the Oklahoma state department of health;
5. The applicant shall secure all easements and permits necessary for construction of the facility in the name of the city;
6. Construction shall be performed by the city/authority unless otherwise agreed in accordance with section 12-916 of this chapter; and
7. Construction will not begin until the applicant deposits with the city/authority an amount equal to the cost of materials for an equivalent water main extension of eight inches (8") in diameter, based upon the latest city/authority materials cost. (Ord. 1354, 8-9-1994)
