901.07 REPAIR OF STREETS OR RIGHTS OF WAY AFTER UTILITY WORK.
   The following procedure shall be followed in performing work within the City streets and controlled rights of way:
   (a)   The utility company shall provide a written notice to the Street Department forty- eight hours prior to any work being performed. In case of an emergency, a FAX transmission to City Hall would be acceptable.
   (b)   The utility company will post a $50,000 Standard Bond (DOT format).
   (c)   All streets are to be patched as follows:
      (1)   Concrete Streets - Patch to be 12" thick over 12" of stone. The existing pavement shall be under cut by 6" deep and 6" back in all directions. Reinforcing shall be 1/4" bars in bottom and 1/4" bars in the top of the patch. Bars to be 2" from concrete surface.
      (2)   Concrete Streets/Asphalt Overlay - Patch as subsection (c)(1) above and 1" full width asphalt overlay street 25 feet in both directions of patch. Total length of patch shall be patch length plus 50 ft.
      (3)   Asphalt Streets - 5" of asphalt patch over 12" of stone fill with full width asphalt overlay as detailed in subsection (c)(2).
   (d)   All restoration shall be completed within one week of repair unless a written extension is given by the City of Dunbar.
   (e)   For winter repairs - All patches to be concrete. The City will submit a spring asphalt paving schedule. Paving to be complete within thirty days of receipt of notice.
   (f)   Work will not be considered satisfactory until a written release is secured from the City.
   (g)   The bonding company will be furnished a monthly report on all unsatisfactory work. After sixty days, the bonding company will be asked to pay for the appropriate corrective action.
