Each application for an excavation permit shall state the name of the person desiring to make such excavation, the purpose for which the excavation is to be made and the person for which the excavation is to be made, the location of the cut in the street, the house number and street name or lot and block number and name of addition, and type of street surface to be cut. A two hundred dollar ($200.00) deposit shall be required for all excavations, including, but not limited to, excavations in concrete pavement or sidewalk, bituminous concrete, brick pavement or sidewalk, tarvia or gravel street, or a dirt road.
Provided, however, if a person or business entity has been a registered contractor with the city of Dixon building department for a period in excess of one year and maintains said registration in accordance with title 7, chapter 1 of this code, said person or business entity may pay a five hundred dollar ($500.00) deposit for multiple openings, not to exceed eight (8) in number at one time.
