1315.02 COMMENCEMENT OF APPEAL.
   (a)    Appeals shall be commenced by the filing of a written Notice of Appeal with the Clerk of the Board of Building and Zoning Appeals. The Notice shall:
      (1)    Set forth the name, address and telephone number of the appellant;
      (2)    Identify the permit or certificate denied or revoked or the Stop Work Order or Order for remedial action to be appealed;
      (3)    A statement of the error alleged in the action of the Commissioner or the grounds asserted in mitigation or as justification for amendment or reversal of the Order.
   (b)    The Notice of Appeal shall be filed within ten days following the date on which issuance of the permit or certificate sought is denied or within ten days following service on the appellant of the notice of revocation of registration, stop work order or Notice and Order to remedy deficiencies from which appeal is taken. Failure to timely file the Notice of Appeal shall render the action of the Commissioner final.
