921.22 PROCEDURE FOR TERMINATION OF SERVICE.
   (a)    When charges for utility services furnished any premises become delinquent, the Utility Billing Manager shall initiate termination of all utility services to the premises by mailing a notice of termination to the premises. If the premises are not owner occupied and the owner has filed a request to be provided with a copy of any notice of termination issued to said premises on the form prescribed by the Utility Billing Manager, a copy shall be forwarded to the owner as directed by the instructions set forth in the request.
   (b)    The notice of termination shall contain the following information:
(1)    The amount to be paid;
(2)    The date the notice was issued;
(3)    The date on which termination of utility services shall occur. The date shall be one on which the Utility Billing Office is open for business not earlier than 10 nor more than fifteen calendar days after the date on which the notice is issued;
(4)    A statement that utility services will be terminated, unless the Utility Billing Office receives full payment of the amount shown to be due prior to the date of termination;
(5)    A statement that services will not be terminated if a deferred payment plan is established pursuant to Section 921.23 prior to the date of termination; and
(6)    The telephone number of the Utility Billing Office and the name of a person to contact regarding the billing charge.
   (c)    The Utility Billing Manager shall terminate utility service to the premises between the hours of 8:30 a.m. and 4:30 p.m. on the date of termination set forth in the notice unless full payment of the amount shown to be due on the notice or a deferred payment plan is established in accordance with Section 921.23 prior to the date of termination set forth in the notice.
