735.03 PERMIT APPLICATION; FEES.
   (a)    Applications for permits required by this chapter shall be made upon a blank form prepared by the Chief of Police and available through the Police Division. Applicants shall complete the form as fully as possible. Applications shall be provided upon request.
   (b)    The form shall provide the following information:
(1)    The name, home address, business address and telephone number at which the applicant can be reached for calls;
(2)    A description of the wrecker vehicle and the hourly availability of the wrecker operator;
(3)    The location of the place where the wrecker operator intends to store disabled motor vehicles, and the means employed to protect the vehicle owner against the damage, destruction or theft of his or her vehicle while in the operator's possession; and
(4)    Such other information as the Chief of Police finds reasonably necessary to effectuate the purpose of this chapter.
   (c)    Upon the recommendation of the Chief of Police, Council will establish a reasonable fee to cover the cost of processing permit applications and issuing permits.
