A. Application:
1. Form: Every application shall be made to the city clerk on a form provided by the city clerk.
2. Information: The applicant shall provide all information required by the application form.
3. False Statement: No person shall make a false statement in or concerning any application. (1982 Code § 7.112)
B. Multiple Devices:
1. Single License: When multiple tables, machines or other devices of one kind are licensed at one place of business, the city clerk may issue one license for all of them and indicate on the license the number of them licensed under the license.
2. Replacement Devices: The licensee may replace any device with another device without the necessity for transfer of the license.
3. Additional Devices: If the license adds to the number of devices used during the license year, a new license shall be obtained for the additional devices.
4. Separate Licenses: Upon request of the licensee or at the discretion of the city council or the city clerk, a separate license may instead be issued for each device. (1982 Code § 7.119)
