A. Application Required: All applications for water or sewer construction permits shall be made to the city clerk by the person employed to do the work before beginning the work involved.
B. Form: The application shall be made in the form prescribed by the city clerk.
C. Required Information: The applicant shall give the legal description of the premises and shall state whether an assessment on the subject premises has been made for water service and for sewer service, and, if so, the amount so assessed.
D. Plan: The applicant shall submit with the application a plan showing the course of the service pipe or drain between the connection with the city water or sewer line and the buildings to be served by it.
E. Additional Plans And Drawings: The applicant shall submit such other drawings and plans of the proposed construction as may be required. (1982 Code § 3.162)
