A. Permit Required:
1. Application: Any person desiring to use public property shall apply for a permit or renewal of a permit a minimum of two (2) working days before starting work and must submit detailed plans for street or sidewalk use and pedestrian safety on major projects.
2. Waiver: This provision or portions thereof may be waived by the director of public works in the event of an emergency.
B. Insurance:
1. Insurance Required: The permittee shall obtain liability insurance for both personal injury and property damage in an amount not less than one million dollars ($1,000,000.00).
2. Additional Insured: The city shall be named as an additional insured under that insurance for the services provided under the permit. The permittee's insurance will be the primary insurance for the city.
3. Certificate Of Insurance: Permittee shall provide a certificate of insurance on the city's approved form which verifies the existence of the required liability insurance coverage as well as workers' compensation coverage.
C. Display Of Permit: Permits issued under this section by the department of public works shall be conspicuously displayed at all times for ease of inspection on the indicated work site.
D. Double Fee: Should any person begin work of any kind without having first secured the necessary permit therefor, such person shall be required to pay double the fee provided for such permit.
E. Revocation: Any permit issued by the director of public works under this section may be revoked when he finds revocation in the best interests of the city. (Ord. 225, 9-5-1995)
