Each affected person shall file, with the city clerk, a statement indicating all current employment and all employment for the previous calendar year of the affected person and his or her spouse. Said statement shall include the name of the employing entity and the nature of the service rendered in the course of such employment.
The statement shall further disclose whether the affected person directly or indirectly performed any service for compensation for the city other than that performed in his official capacity with the city, or was associated with or employed by a corporation or partnership which received compensation for services performed for the city.
