3-8-6: APPLICATION FOR MASSAGE THERAPIST LICENSE:
   (A)   An application for a massage therapist license shall be filed with the city clerk of the city of Darien. The application shall be made upon a form provided by the city clerk and shall be completed in full and signed by the applicant, verified by oath or affidavit, and shall set forth: (Ord. 0-39-00, 12-18-2000, eff. 2-1-2001)
      1.   If employed by a massage establishment, the business address and the applicant's home address and telephone numbers where the applicant will be providing massages. If not employed by a massage establishment, the applicant's home address and telephone number. (Ord. 0-01-02, 1-21-2002)
      2.   The full name, home address with zip code, social security number, driver's license number, date of birth, sex and a physical description of the applicant, including height, weight, color of hair and eyes.
      3.   The business, occupation, and employment of applicant for three (3) years preceding the date of application. (Ord. 0-39-00, 12-18-2000, eff. 2-1-2001)
      4.   Whether the applicant ever made an application for a massage therapist license under this chapter or a massage therapist's or similar license to any state, county, city, village or other unit of local government, and if so, where and when and if such application was granted or denied, and if such application was granted or denied, and if such application was denied, the reasons for the denial.
      5.   Whether a massage therapist license was ever issued to the applicant under this chapter or a massage therapist's or similar license was ever issued by any state, county, city, village or other unit of local government, and if so, where and when and if such license has ever been suspended or revoked and the reasons for the suspension or revocation.
      6.   Whether the applicant has ever been convicted of a violation of any provisions of this chapter or any ordinance of any other Illinois municipality which regulates massage establishments or the giving of massages, or any Illinois statute regulating massage therapists.       7.   Whether the applicant has ever been convicted of a violation of any "Sex Offenses" as delineated under 720 Illinois Compiled Statutes 5/11-5 through 5/11-22 or any equivalent foreign state statute or federal law within five (5) years of the date of the application.
      8.   Whether the applicant has ever been convicted of the commission of a felony under the laws of this state, or any other state, or under the laws of the United States.
      9.   Proof that the applicant is a "massage therapist", as defined in section 3-8-1 of this chapter.
      10.   Proof that the applicant currently carries professional liability insurance in an amount of no less than one million dollars ($1,000,000.00) per occurrence.
      11.   Proof that the applicant is currently certified in cardiopulmonary resuscitation (CPR).
   (B)   The applicant shall submit a statement in writing from a physician duly licensed in the state of Illinois that he or she has examined the applicant and believes the applicant to be free of all communicable diseases. An applicant is required to undergo an annual physical examination and to furnish a medical certificate relating to communicable diseases with his or her annual fee.    (C)   The applicant shall submit a complete set of fingerprints with the application for a massage therapist license. All such fingerprinting shall be done by the city of Darien police department. Said fingerprints shall be submitted to the appropriate state and/or federal agencies for processing as available. The cost of fingerprinting shall be paid by the applicant in addition to any other required license or application fee. After the initial application for a license, fingerprints for the renewal of the license shall be required every three (3) years unless the license has lapsed or has been revoked. If the license has lapsed or been revoked, the applicant shall be required to submit a new (initial) application with a complete set of fingerprints. (Ord. 0-01-02, 1-21-2002)
   (D)   The applicant shall submit a written authorization for the city, its agents and employees to seek information and conduct an investigation into the truth of the statements set forth in the application and the qualifications of the applicant for a license.
   (E)   The applicant shall submit such other information, documentation, and identification of the applicant as the director of community development and/or the chief of police shall deem necessary to determine the identity of the applicant or to process the application.
