Application for a solid waste collector permit or its renewal shall be made upon a form prepared and furnished by the County Manager. The application shall include the following information:
(A) Brief description of facilities, equipment and proposed operating methods of the applicant;
(B) Indication of the geographic area proposed to be served by the applicant;
(C) Number of employees; and
(D) Evidence of financial responsibility commensurate with the service to be rendered.
