An application for a permit shall be made in writing to the Mayor. The application shall state the location of the proposed well, the owner of the site, the lessee, if any, the person to be in charge of the drilling operations, the approximate date of commencement of operations, the estimated period for the completion thereof and a description of the equipment to be used.
The application shall contain an agreement that in the drilling of the well, and in the restoration of the premises, all requirements of the ordinances of the Village then in effect will be fully and strictly complied with. The application shall be signed by the owner of the premises and by the lessee, if any, and shall be accompanied by a certificate of the Building Commissioner that all equipment proposed to be used in the drilling operations conforms to the requirements of this chapter.
