§ 125.07 REPORTS.
   (A)   It shall be the duty of every person conducting or operating any home regulated by this chapter to make a report to the Mayor or his or her designated village employee or inspector, by telephone and confirmed in writing immediately thereafter, of any outbreak of epidemic, communicable or contagious diseases. OUTBREAK means the occurrence of disease or illness that requires a report to any federal, state or Cook County agency.
   (B)   It shall be the duty of every person conducting or operating any home regulated by this chapter to report to the Village Police Department by telephone any unusual occurrences, including, but not limited to: 1) missing persons, 2) unaccounted for or the disappearance of controlled substances and syringes, 3) incidents resulting in severe personal injury or death due to resident altercations, 4) attempted suicides, suicides, and sudden unexplained deaths, and 5) theft or stolen property with a value in excess of $100. A copy of an email to any agency of the State of Illinois shall suffice in such circumstance.
