§ 125.06 RECORDS.
   (A)   Resident’s records.
      (1)   Individual records. Each home regulated by this chapter shall maintain individual records for each resident and shall include the resident’s admission record (which record shall show the name, address, age, sex, marital status, the date of admission and discharge or disposition of each resident in the home), a medical record, a physician’s order record, nursing notes, and a list of the resident’s belongings.
      (2)   Permanent residents’ registry. Each home regulated by this chapter shall maintain a permanent residents’ registry book or comparable electric archive in which the name of each resident shall be entered in the chronological order of admission.
      (3)   Financial transactions with resident’s funds. Each home regulated by this chapter shall maintain accurate records of financial transactions involving the use of each resident’s funds.
      (4)   Death records. Each home regulated by this chapter shall maintain a written or electronic record showing the name of each person who dies while a resident therein, the date of death, the name and address of the person to whom the body is released, the date of removal from the home, or if finally disposed of by the home, the date, place and manner of disposition.
   (B)   Personnel files. Each home regulated by this chapter shall maintain accurate personnel files for all employees which shall include records of education, experience, findings of medical examinations as to physical and mental fitness. Daily work and time schedules of employees shall be maintained as part of such personnel files.
   (C)   Inventory of controlled substances. Each home regulated by this chapter which dispenses, administers or distributes any controlled substances shall maintain an inventory of such controlled substances in accordance with the federal and state law and rules promulgated by the Illinois Department of Registration and Education.
   (D)   Open to Mayor or his or her designated village employee or inspector. All records shall be open at all reasonable times for inspection by the Mayor or his or her designated village employee or inspector or his or her duly authorized representative.
