§ 119.51 APPLICATION OF LICENSE; AFFIDAVIT; PHOTOGRAPHS GENERALLY.
   (A)   Any person desiring to secure a village taxicab driver’s license shall make application in writing therefor to the Mayor upon a form to be provided by the Village Clerk. The application shall set forth the name of the applicant, his or her residence and occupation and shall be endorsed by at least two responsible citizens who shall certify that the applicant is a person of good habits, honest, sober and industrious and a fit person to drive or operate a taxicab.
   (B)   Along with the application, the applicant shall file an affidavit stating the applicant’s full name, residence, places of residence during the previous five years, age, sex, race, height, color of eyes and hair, place of birth, whether a citizen of the United States, places of previous employment, whether he or she has ever been convicted of a felony or misdemeanor, whether he or she has previously been licensed as a driver or chauffeur and if so, whether his or her license has ever been revoked and for what cause, which affidavit shall be filed with the Village Clerk as a permanent record.
   (C)   The applicant shall also file with the application to the Village Clerk two recent photographs of himself or herself of a size which may be easily attached to the license, one of which shall be attached to the license when issued and the other shall be filed with the application in the office of the Village Clerk.
   (D)   Each applicant for a taxicab driver’s license must:
      (1)   Be of the age of 18 years or over;
      (2)   Be of sound physique with good eyesight and not subject to epilepsy, vertigo, heart trouble or any other infirmity of body or mind which might render the applicant unfit for the safe operation of a taxicab;
      (3)   Be able to read, write and speak the English language; and
      (4)   Be clean in dress and person and not be addicted to the use of narcotics or intoxicating liquors, as determined by a drug and alcohol testing as part of the driver interview and screening process.
