The Safety-Service Director is hereby authorized to issue permits as required by Section 721.01 on the following conditions.
(a) In order to protect residents from the inherent damages from excess competition, not more than three such permits shall be in existence at one time.
(b) No permit shall be valid for more than one year. Provided, however, that any permit now in existence or which may be hereafter issued may be renewed by the payment of the annual fee unless the Safety-Service Director has reason to believe that the permit holder no longer qualifies for permit.
(c) Applicants shall furnish necessary evidence of his ability to perform the service for which such permit is to be issued. Included in such information will be the name and address of applicant and all employees of applicant with a statement as to whether either applicant or any of his employees has ever been convicted of a crime, and if so, the nature of and detailing thereof and a list of all equipment, including make and year of truck or trucks and experience record which would qualify the applicant to perform the services for which such permit is issued.
(d) On request of the Safety-Service Director, the applicant shall furnish financial information so far as is necessary to show financial ability to purchase necessary equipment and perform the services for which a permit is issued.
