To obtain a permit, the applicant shall first file an application in writing on a form furnished by the Code Enforcement Agency for that purpose. Every applicant shall:
(A) Identify and describe the work to be covered by the permit for which application is made.
(B) Describe the land on which the proposed work is to be done by street address or similar description that will readily identify and definitely locate the proposed building or work.
(C) Indicate the use or occupancy for which the proposed work is intended.
(D) Be accompanied by plans, diagrams, computations and specifications and other data as deemed necessary by the Building Commissioner.
(E) State the valuation of any new building or structure or any addition, remodeling or alteration to existing building.
(F) Be signed by the applicant, or the applicant’s authorized agent.
(G) A copy of the Montgomery County Drainage Board’s Approval of the Drainage Plan where applicable.
(H) A copy of a design release, when applicable, issued by the State Building Commissioner and the State Fire Marshall pursuant to IC 22-15-3-1.
