Permits shall contain, as appropriate, the following:
(A) Statement of duration, including issuance and expiration dates;
(B) Effluent limitations, including best management practices based on the more stringent of categorical pretreatment standards, local limits as established by this subchapter, and state and local law;
(C) General and specific discharge prohibitions as established by § 52.128 of this subchapter;
(D) Requirements to pay fees for the wastewater to be discharged to the POTW;
(E) Limitations on the average and maximum rate and time of discharge or requirements for flow regulation and equalization;
(F) Statement of self-monitoring requirements;
(G) Requirements for installation and maintenance of inspection and sampling sites;
(H) Requirements and specifications for monitoring programs including identification of pollutants to be monitored, sampling locations, frequency of sampling, sample type based on federal, state, and local law, standards for tests and reporting schedule;
(I) The process for seeking a waiver from monitoring for a. pollutant neither present nor expected to be present in the discharge. Once waived, a list of specific waived pollutants shall be included within the permit;
(J) Compliance schedules;
(K) Requirements for submission of technical reports, notifications, discharge reports or certification statements. These include any reporting requirements contained in a national categorical standard or pretreatment requirements;
(L) Statement of requirements for collecting/retaining and providing access to plant records relating to the user’s discharge and for providing entry for sampling and inspection;
(M) Requirements for notification of any new introduction of wastewater constituents or any substantial change in the column or character of the wastewater treatment system;
(N) Requirements for notification of spills, potential problems to the POTW including slug loadings, upsets or violations;
(O) Requirements for installation, operation and maintenance of pollution control equipment;
(P) Requirements to develop and implement spill and slug control plans;
(Q) Other conditions as deemed appropriate by the POTW to ensure compliance with this subchapter, state and federal pretreatment standards and requirements;
(R) Statement of applicable civil and criminal penalties for violation of pretreatment standards and requirements;
(S) Statement of non transferability;
(T) Conditions for modification or revocation of permit; and
(U) Requirements and conditions in this section are pursuant to 40 CFR § 403.8.
