§ 119.025 DRIVER’S IDENTIFICATION CARD.
   (A)   Identification card application. No person shall drive or be in actual physical control of a taxicab within the limits of the city unless he or she is in possession of a valid driver’s identification card. The driver of a taxicab must apply for a driver’s identification card from the Finance Department. The application for a driver’s identification card shall be filed with the Finance Department on forms provided by the city and such application shall be verified under oath and shall contain and be accompanied by the following:
      (1)   The applicant’s name, Social Security number and date of birth;
      (2)   The applicant’s residence address and telephone number and states of residence during the three years preceding the date of the application;
      (3)   The applicant’s state-issued motor vehicle operator’s license number, original date of issue and date of expiration;
      (4)   A description of the applicant’s experience in driving motor vehicles generally and in driving taxicabs;
      (5)   The name, address and telephone number of the owner of the taxicab to be driven by the applicant;
      (6)   The taxicab number of the taxicab to be driven by the applicant;
      (7)   A statement that the applicant has read and understands the provisions of this subchapter applicable to drivers of taxicabs; and
      (8)   Any additional information required on the application form prescribed by the Finance Department.
   (B)   Application fee. An application fee of $40 shall be charged at the time of making the application.
   (C)   Must obtain driver’s identification card from Police Department. The driver must obtain a driver’s identification card from the Police Department. The driver will be fingerprinted by the Police Department before the driver’s identification card is issued.
The driver’s identification card shall bear the name, age and photograph of the applicant and shall state that the driver is certified to drive a taxicab within the city. The driver’s identification card shall be prominently displayed inside the taxicab during all times of operation so that passengers may view the identification card from his or her seat. All drivers must produce his or her driver’s identification card to any peace officer of the city upon request.
