(A) The application for a license required by this subchapter shall be in writing and accompanied by a certified check or cash in the amount of the license fee.
(B) The application shall set forth:
(1) The full name and address of all interested persons therein;
(2) The location and description of the room or premises where the licensed activity will be conducted; and
(3) The number of pool or billiard tables to be used.
(C) On approval of the application by the Finance Department, the Finance Department shall issue the license, but only to the actual proprietor of the licensed business room or premises.
