Every physician, midwife or other person who may professionally assist or advise at any birth shall within five days thereafter make out and present to the Clerk of the Board of Health a written statement of the sex and color of every child born, the names of the parents, and the nativity and social relation of each of them. Every physician and professional adviser who has attended any person at a last illness or has been present at the death of any person shall within twenty-four hours thereafter make out and present to the Clerk of the Board of Health a statement of the death, specifying the cause, date, hour and place of death.
