1723.01 PHYSICIAN’S REPORT REQUIRED.
   Each physician or other person called to attend a person suffering from smallpox, cholera, plague, yellow fever, diphtheria, membraneous croup, scarlet fever, or any other disease dangerous to the public health, or which is required by the State Board of Health to be reported, shall report to the Board of Health of the City. Patient demographics shall be released to the Board per Ohio R.C. 3701.17.
