Every applicant for a mechanical amusement device license shall complete a written application prescribed by the Mayor and submit an application fee of three hundred dollars ($300.00) for each license requested. The application shall provide all of the following information; the name of the device; the name and address of the manufacturer of the device; the model number of the device; the serial number of the device; a photograph of the device; a written description of the manner in which the device is played including the object of the game, contest or amusement; the name, address and social security number of the owner of the device and if different, the applicant. The Mayor may also request that the owner or applicant provide a certified copy of a report prepared by an Independent Testing Laboratory or qualified expert as defined in this Chapter, finding conformity with the Statutes of the State of Ohio and the Ordinances of the City of Coshocton which pertain to gambling, at any time prior to or after a mechanical amusement device license is issued. The report shall include a list of software "signatures" identifying the software evaluated. A supplemental report shall be provided each time any software changes occur in the device. Upon approval of the application, the Mayor shall issue a decal which shall be affixed to the device in a prominent location so that all persons playing the device may see that the device has been licensed by the City of Coshocton.
