The statement of financial interests shall be filed on a form prescribed by the Board of Ethics. The Board, or the designated administrative official, shall deliver a copy of the form to each elected official and candidate required to file the statement, by first class mail or hand delivery, no later than thirty (30) days preceding the date on which the statement is required to be filed. The failure of the Board or the designated administrative official, to deliver a copy of the form to any elected official or candidate shall not relieve the elected official or candidate of the obligation to file the statement.
